This form is to be completed by AUTHORIZED INDIVIDUALS who would like access to the DigitalSandbox (DSBox), and/or Mobile Indicator.
Two User Types: NJEMSTF Leadership & NJ County EMS Coordinators - Full access to DSBox & Mobile Indicator
NJEMSTF Members, wheeled assets & County/Local EMS staff - Mobile Indicator access
Complete the following form to request access. NOTE: OFFICIAL AGENCY EMAIL IS REQUIRED for individual access (County, State, Agency email). No gmail, aol, live, outlook, etc emails will be allowed access.
If you are requesting access for both individual access, as well as Wheeled Asset access, you must submit two separate requests.
